One of our senior residents, a 67 year old African American woman, had not been out of her apartment for two years. This was discovered during one of the wellness checks performed by the current SSC. We were able to get caregivers in to help this resident and registered her with the Cluster program and food bank to get food to this resident who was weighing about 90 lbs. Youth residents also delivered fresh vegetables from the community garden. With the assistance she received and the regular meals, the resident was able to gain 27 pounds and now will take short walks. Recently she ventured out of her office to come and get her Rent Rebate forms completed. It is an amazing transformation for this formerly homebound senior.
